Kidney transplantation and the new allocation programme 26 VI–31 XII 2008

Abstract

The aim of this study was preliminary evaluation of the selection of the recipient based on the new allocation programme compared to the previous system. Allocation with the new rules began on 26[sup]th[/sup] of June, 2008 at the Immunohistocompatibility laboratory of the Department of Clinical Immunology of Medical University in Warsaw. Regional HLA laboratories began their work in the new allocation programme in the last quarter of the 2008.
Results: In the period between 26th of June and the end of December, 2008
- 411 kidney recipients were transplanted. 285 was chosen based on the new
allocation programme (group I), 126 was selected in the regional transplant
centres according to the previous rules of allocation (group II). When analysing the HLA compatibility in both groups of the transplanted recipients, it was determined, that the selection made according to the new allocation rules allowed to transplant a kidney with a 15% better match. The dialysis time in both groups was not different signifi cantly and amounted at an average of 2.5 years. When evaluating the selection of recipients according to their place of residence, it was determined, that in group I - 35.7% was regional, and in group II - 87%.
Conclusions: 1. New allocation system better allows righteous and according to the transplantation society rules selection of the kidney recipient.
2. Compliance to the rules of the new allocation system when selecting a
kidney recipient makes necessary to obtain the serum of all registered and
qualified for transplantation potential recipients from the whole country.
3. Until the planned redistribution of serum of all potential recipients to the regional HLA laboratories will be implemented, it is necessary to make a cross-match simultaneously at both regional HLA laboratory and in the Central laboratory in Warsaw.
