Neutrophils degranulation marker – leucocyte elastase-α1-protease inhibitor complex: comparative study with classic (α1M, ß2M) and novel (Ngal, Il-18) kidney graft injury markers

Abstract

Background: Serum and urine α1M, ß2M as well as NGAL and IL-18 are
established markers of kidney graft function. The study's aim was to seek correlations between neutrophils activation (LEα[sub]1[/sub]-PI complex) and classic as  well as novel kidney graft tubulo-interstitial injury markers measured in blood and urine of deceased renal allograft's recipients.
Material/Methods: The study was performed on 24 kidney graft recipients. Using ELISA test plasma and urine LE-α[sub]1[/sub]-PI was assessed after kidney transplantation (KTx) - on postoperative (PO) days 1[sup]st[/sup], 3[sup]rd[/sup] and 7[sup]th[/sup]. Serum and urine NGAL, IL-18 and b2M were measured by ELISA test and α[sub]1[/sub]M - by  nephelometric method on 1[sup]st[/sup], 3[sup]rd[/sup], and 7[sup]th[/sup] PO day.
Results: Correlation was found between plasma LE-α[sub]1[/sub]-PI and serum NGAL on 1[sup]st[/sup] and 3rd PO day (p<0.02 and p<0.002 respectively). Urine LE-α[sub]1[/sub]-PI on 1[sup]st[/sup] PO day correlated with serum and urine NGAL on 3[sup]rd[/sup] and 7[sup]th[/sup] PO day (p<0.03; p<0.001; p<0.009 and p<0.001 respectively).There was no important
association between LE-α[sub]1[/sub]-PI and IL-18 level in serum and urine.Plasma LE-α[sub]1[/sub]-PI on the 1[sup]st[/sup] PO day was correlated with serum α[sub]1[/sub]M on 3[sup]rd[/sup], 7[sup]th[/sup] PO day Urine LE-α[sub]1[/sub]-PI on 1[sup]st[/sup] PO day correlated with serum α[sub]1[/sub]M on 1[sup]st[/sup], 3[sup]rd[/sup] and 7[sup]th[/sup] PO day and with urine α[sub]1[/sub]M on 7[sup]th[/sup] PO day as well. Similar associations were found between LE-α[sub]1[/sub]-PI on 1[sup]st[/sup] PO day and ß2M.
Conclusions: Neutrophils degranulation measured by plasma and urine LE-α[sub]1[/sub]-PI complex level was correlated with kidney graft tubulo-interstitial  damage
markers in the early period after KTx.
